Heart and blood sugar: an underestimated connection that research is finally addressing
There are connections between diseases that medicine has known about for decades, yet in daily clinical practice they are still rarely treated together. Coronary heart disease and impaired blood glucose regulation are one such pair. They are not two separate problems that happen to occur in the same person by chance: they share common biological roots, feed into each other, and if not addressed together, tend to worsen in tandem.
The data is uncomfortable but precise: approximately two thirds of patients with coronary heart disease also have diabetes or prediabetes. Yet the prediabetic phase, when glycemic values are already altered but have not yet reached the threshold of full-blown diabetes, is often left without targeted pharmacological treatment. Guidelines recommend lifestyle changes, but anyone working in a clinical setting knows how difficult these are to maintain over time.
This is where a study published in Evidence-Based Complementary and Alternative Medicine raised a concrete question: can Ginkgo biloba offer real therapeutic support in these patients?
A plant with a precise pharmacology
Ginkgo biloba is not simply a “memory plant,” as it is often reductively described. Its active components, primarily flavonoids and terpene lactones, have documented effects on multiple fronts: they improve blood flow, counteract oxidative stress, and modulate inflammatory processes.
What emerges from more recent literature, however, concerns a lesser-known mechanism: its ability to act on insulin resistance. Ginkgo biloba extract (EGb) displays an interesting and in some ways unusual behaviour: in patients with hyperinsulinemia it reduces insulin levels, while in patients with impaired beta-cell function it stimulates secretion. A bidirectional adaptation that makes it potentially useful across different metabolic profiles.
Preclinical studies have shown significant reductions in fasting blood glucose, improved insulin sensitivity, and protection of pancreatic cells from oxidative stress, a coherent biological basis that justifies clinical interest.
Why this study is different
The protocol chosen by the researchers is not the classic large-scale trial. It is an N-of-1 trial, an experimental design in which each patient alternates periods of treatment with Ginkgo biloba and periods with placebo, effectively becoming their own control group. According to Prof. Gordon of McMaster University, this approach sits at the top of the clinical evidence pyramid, precisely because it eliminates confounding variables linked to individual differences that in traditional RCTs can mask real responses.
For a chronic condition like coronary heart disease with prediabetes, where therapeutic response varies considerably from patient to patient, it is a methodological choice with a precise logic. The observation period extends over 58 weeks, with periodic assessments of fasting and postprandial blood glucose, quality of life, glycated haemoglobin, lipid profile, C-reactive protein, and metabolomic analysis.
The question that really matters
What this research puts on the table is not merely the evaluation of a plant. It is a broader question: does a natural, safe, and long-term sustainable approach exist for the cardiological patient with prediabetes?
The pharmacological options available for impaired glucose regulation (IGR) are few, and not always compatible with the polypharmacy already in place in these patients. A botanical extract with decades of clinical use behind it, a well-established safety profile, and a pharmacology that touches both glucose metabolism and cardiovascular health responds to a real need that conventional medicine has not yet resolved satisfactorily.
The trial results will provide more definitive answers. But the direction already indicated by existing literature is sufficiently solid to justify close attention.
